“Beyond NO blame NO shame: How to implement patient safety in MSF field projects!”

Why this debate?

Quality of care is at the core of our medical services. OCB has over the past decades look at different elements of improvement and strengthening the quality of care in project in the field. Last motion from  2021 A common framework for reporting quality of care in MSF 

Patient safety is one of the pillars that have been under focus during improvement of quality of care. Good patient safety measure will ensure that our patient is getting good quality care.  

During the work with patient safety implementation, one has seen that there is many barriers and challenges to implement to the field projects. Our wish with the discussion is to examine different perspectives of patient safety.
The barriers and opportunities and new perspectives towards implement patient safety measure in our field projects.  

Why is difficult to talk about our mistakes? We want to move towards a no shame no blame culture. How to find good ways to discuss these issues in our organization that includes all our staff. A discussion that makes our medical team feels secured but also challenges us to improve patient safety.  

 

 Some elements that have been highlighted  

  • Technically, our current reporting system lacks user-friendliness, is time-consuming, and heavily relies on individual effort rather than system efficiency.  

  • Culturally, the prevalence of blame within OCB and other OCs remains a significant issue. There is still a lot of vagueness surrounding the definition of negligence and the direction between incidents managed as learning experiences versus those involving HR, which undermines staff confidence and slows down cultural change.  

  • Legally challenges and barriers in different context that cause challenges to implement. 

  • What is the ethically consideration to think of when implement patient safety?
